Job Title: Case Manager - Shelter Support
Classification: Non-Exempt / Full-Time (40 hours per week)
Reports to: Program Manager
Pay: $18 - $20/hr
Hours: M - F 9am - 6pm

OVERVIEW: Local non-profit that assist it's communities to address the needs of low-income, homeless and under-served people in North San Diego County.

WHAT YOU WILL DO: This position will provide direct services to clients, to include but not limited to, intake, assessment and case management, and client advocacy (resources and referrals) to homeless and low-income individuals, with the goal of moving clients from crisis to self-sufficiency and to support our mission. This position provides case management support to clients residing in temporary shelter.


*   Provides direct services to clients including diversion, Individual Service Plan (ISP), assessment, crisis intervention, advocacy, and case management
*   Provides guidance and support to help clients identify their own strengths, set achievable goals and follow through consistently so they can become self-sufficient
*   Utilizes Housing First and Diversion strategies to stabilize clients experiencing or at-risk of experiencing homelessness
*   FACILITATES ACCEPTANCE OF PLACEMENT OR OTHER SERVICES THAT SUPPORT AND STABILIZE INDIVIDUALS MOVING FROM TEMPORARY SHELTER
*   Identifies resources and advocates for clients' needs
*   Links and networks clients with resources within the agency and utilizes other public and private social service agencies as necessary
*   Ability to work independently and professionally at partnering agencies, faith centers and other community settings.
*   Ability to work with and collaborate with volunteers, interns and other agency personnel.
*   Provides services in the community; working with clients in various settings, which include but are not limited to, other agency programs, faith centers, and school administrations
*   Responsible for meeting specific outcomes per month required by program contracts
*   Enters data in multiple databases and tracking tools
*   Participates in all staff meetings and trainings
*   Performs additional duties as assigned

EXPERIENCE / EDUCATION
*   1+ years of work experience in a social services or nonprofit setting required
*   Bachelor's Degree in Social Services or related field; equivalent combination of work experience and education may be considered
*   Experience working with homeless population preferred
*   Experience providing case management preferred

SKILLSETS / ADDITIONAL REQUIREMENTS
*   Ability to establish and maintain professional boundaries with clients
*   Must have excellent written and verbal communication skills
*   Must be highly organized and demonstrate attention to detail
*   Experience writing accurate and comprehensive case notes


*   Demonstrated  experience  managing  multiple  deadlines  and  working  effectively  under pressure, using independent judgment to produce quality work under time constraints
*   Demonstrated experience working with a collaborative team
*   Working Knowledge of MS Office (Word, Excel, and Outlook)
*   Ability to communicate effectively among  a diverse workforce population  and maintain sound working relations with all levels within the organization
*   Work with spirit of enthusiasm, teamwork, cooperation and a sense of urgency
*   Maintain  a  high  degree  of  confidentiality  over  all  matters  in  the  course  of  business operations including client and employee information
*   Ability to multi-task in an efficient, thorough, and prioritized manner
*   Ability to work quickly, accurately and independently
*   Ability to anticipate needs, prioritize work and solve problems
